Making A Donation By Mail |
If you would like to make a donation by mail, you can do
so in either of the following ways:
- Send a check made payable to the Arizona Institute for
Breast Health
- Send your credit card information in the mail - we accept Visa, MasterCard, Discover and American Express

When sending a check in the mail, please
be sure to include the following information:
- Mailing address and phone number of donor
- A second name if the donation is in memory or honor of another
|
When sending your credit card information in the mail, please
be sure to include the following information:
- Credit card number, expiration date, security code and billing address
- Name as it appears on the credit card
- A second name if the donation is in memory or honor of another
